Contact and Intake Interest Forms
When you submit a contact or appointment interest form on our site, we collect your full name, email address, phone number, date of birth, and any message you include. This information is used solely to respond to your inquiry or schedule an introductory appointment. It is not used to create or update a medical record.
Enrollment
Patient enrollment is handled through a third-party platform (Hint Health) linked from our site. Any information submitted through that platform is governed by Hint Health’s own privacy policy and terms of service.
Google Analytics
We use Google Analytics (GA4) to understand how visitors use our website. GA4 collects anonymized data including pages visited, time on site, general location, device type, and browser. This data is processed by Google and governed by Google’s Privacy Policy. We do not use this data to identify individual visitors. You can opt out via the Google Analytics Opt-Out Browser Add-on.
Website Technical Data
Like most websites, our site automatically collects basic technical data when you visit, including your IP address and browser type. This is used for security and spam prevention only.
How We Store Your Data
Contact form submissions are received via email and managed internally by Alpenglow Family Medicine staff. Your data is stored on password-protected systems and is not uploaded to third-party platforms.
Who Has Access to Your Data
Your personal information is not sold or shared with third parties. It is accessible only to Alpenglow Family Medicine staff directly involved in responding to your inquiry.
Important Note on Medical Records and Health Information
This privacy policy covers only data collected through this website. The collection, use, and protection of your medical information as a patient of Alpenglow Family Medicine is governed separately under HIPAA and our Notice of Privacy Practices, available upon request at our office.
How Long We Retain Your Data
We retain contact information for as long as necessary to fulfill the purpose for which it was collected. To request deletion, contact us at memberservices@alpenmd.com or 719-260-1161.
Your Rights Under the Colorado Privacy Act
As a Colorado resident you have the right to know what personal data we hold about you, access or correct that data, and request its deletion. To exercise these rights, contact us at memberservices@alpenmd.com. We will respond within 45 days.
